Ulu Laukahi Project (ULP)

Project Description:

Five Mountains Hawai’i, Inc. will increase access to holistic, managed care and support to improve Native Hawaiian physical and mental health, including the chronic disease conditions of obesity, hypertension, diabetes, anxiety, and depression. Currently, the Western silo approach to medical treatment separates the various chronic conditions of obesity, hypertension, diabetes, and mental health, treating them in isolation rather than holistically, which ignores the interconnection between these chronic disease conditions and the need to treat these conditions comprehensively. By implementing a culturally informed prevention and intervention program that provides comprehensive support, each patient’s medical and behavioral health needs will be addressed.  This project aims to ensure Native Hawaiians live long, healthy lives and have a strong sense of well-being.
